A Universal Child Health Record signed by your physician with immunization records, proof of a yearly flu vaccine, and allergy or asthma plans if needed. Everything is due by August 1.
No. Staff do not administer medication. Allergy treatment requires a physician's plan, and EpiPens must be unopened in their original containers.
